This is the mechanism to share all needed documents and resources for the MOC IV Project: Assessing and Improving Physician Wellness. This activity will not award any CME credits or MOC points. The learner must complete the the QI interventions and submit appropriate data for approval to the MOC Portfolio Manager, Dr. Pardue. Once Dr. Pardue approves the appropriate criteria have been met, the learner will then submit an Attestation Form for QI projects from the ABP website to the MOC Portfolio Administrative team.

The following attachments and resources are necessary to successfully complete the Assessing and Improving Physician Wellness MOC IV project. Please download and save all items in an easily accessible place for future reference. Viewing and downloading the attachments do not constitute receiving ABP MOC Part 4 points. Please complete the quality improvement project in its entirety, then submit collected data to the ABP MOC IV Portfolio Manager for review and approval. Once approved, complete the ABP Attestation Form for QI Projects. Instructions for this process are included in the PowerPoint (slide 19). Learners will receive an email from the ABP once MOC Part 4 points are fully awarded.
